New Business Packet Request Form

The new business packet is designed for businesses that have recently opened or are getting ready to open in North Carolina that employ more than five workers. The packet includes information about some of the services we provide. If you would like us to mail you a new business packet, please complete the form below and submit it to the N.C. Department of Labor. Please note that if you have only one employee or are an out-of-state business, we must e-mail you a new business packet. NOTE: All items in red are required fields.
